The Elder Care Support Program

Navigating aged care services can feel overwhelming, especially when trying to understand what support is available and how to access it. The Elder Care Support Program is here to make this process simpler, helping elders and their families connect with essential services provided by My Aged Care to maintain independence and stay connected with their community.

What is the Elder Care Support Program?

The Elder Care Support Program aims to support elders in understanding and accessing My Aged Care’s in-home services, tailored to meet personal needs and goals. My Aged Care provides various services designed to support individuals to live independently, safely, and comfortably in their own homes. Through the Elder Care Support Program, individuals can find out what options are available, learn how to apply, and receive ongoing support throughout the process.

Am I Eligible for the Elder Care Support Program?

To find out if you qualify for the Elder Care Support Program, consider the following questions:

Do you identify as Aboriginal or Torres Strait Islander? And;

  • Are you over 50 years old?
  • Do you have limitations on what you can do physically?
  • Are you experiencing increased forgetfulness?
  • Do you have chronic health conditions?
  • Do you rely on family or friends for more assistance than before?
  • Have you experienced any recent falls?
  • Have you been in the hospital recently or experienced unplanned weight loss?

Answering “yes” to any of these questions could mean you are eligible for support through My Aged Care and can benefit from the guidance provided by the Elder Care Support Program.
