Short Term Care Pathways Under Support At Home

How Brightwater can help

When you or someone you love needs extra help at home, it can be hard to know where to start. The Australian Government’s Support at Home program includes several short-term care pathways designed to give people a boost when they’re going through a change in health, mobility or confidence at home.

These short bursts of support can make a big difference, helping people recover, adjust and stay as independent as possible in their own home.

Brightwater currently offers the following short-term care pathway:

1. Home Modification and Assistive Technology Pathway

Sometimes, small changes at home can prevent big problems later on.

Under this pathway, Support at Home clients with Brightwater can receive support to:

  • Modify your home – for example, adding grab rails in the bathroom, installing a ramp, or improving lighting to reduce the risk of falls.
  • Access assistive technology – such as shower chairs, mobility aids, handrails, or other equipment that makes daily tasks easier and safer.

Our team works with you to:

  • Understand how you move around your home and what’s important to you
  • Identify practical changes that will help you stay safe and independent
  • Arrange quotes and coordinate with trusted tradespeople and suppliers

The goal is simple: to help you stay in the home you love, with the support and tools you need to feel confident.

What Brightwater does not offer right now

2. Short-Term Restorative Care Pathway (STRC)

This pathway is for people who have noticed a recent change in their abilities, perhaps after an illness, a fall, or a stay in hospital, and want to regain strength and independence.

However, it’s important to note that Brightwater at Home does not currently provide the Short-Term Restorative Care pathway under the Support at Home program.

3. End-of-Life Pathway

Under Support at Home, there is also an End-of-Life pathway, designed for people who are in the final stages of life and wish to remain at home with specialised support.

At the time of publishing this article, Brightwater does not offer the End-of-Life pathway under Support at Home.
