If you need a reminder for what happens next, read on.

If you’ve rung about the Commonwealth Home Support Programme (CHSP)

If you’ve given us a ring about the Commonwealth Home Support Programme, its likely to see if we are currently accepting referrals. We accept referrals through the My Aged Care portal and contact you directly to organise your services.

If you need to ring us regarding your CHSP referral, upcoming appointment, or agreement, we can often put you straight through to the team to help with your enquiry.

If you have rung about a Home Care Package (HCP)

If you have rung to register your interest in a HCP, we will often ask you which letter you have received from My Aged Care. The letter we are looking for is the letter explaining you have been assigned a home care package.

This letter will begin with ‘You’ve been assigned a home care package’.

It will also include a referral code in red on page 2. This is the referral code we will use to begin your HCP application.
