End Of Life Pathway Through Support At Home

At this time, Brightwater does not provide the End-of-Life pathway under the Support at Home program.

We understand that end of life care is a very important and sensitive topic, and many people wish to remain at home, surrounded by familiar things and the people they love. While Brightwater currently offers other Support at Home pathways, such as help with home modifications and assistive technology, we are currently developing our approach to end of life support.

Will this change?

Programs and funding in aged care can change over time. If you are interested in Brightwater at Home and want to know whether we now offer the End-of-Life pathway, you are welcome to:

  • Contact Brightwater directly to ask about current services
  • Let us know what kind of support you or your family member are looking for
  • Ask for guidance on who else may be able to provide end of life care at home.

Our team can’t offer the End-of-Life pathway under Support at Home at this stage, but we can:

  • Explain your options in simple terms
  • Point you towards other providers or services that may be able to help.
