Advance care planning allows a person to express their wishes and preferences for their future care and health decisions. Unfortunately, these conversations can be difficult, and many people living at home have not planned for their future health care. Planning ahead can help families make decisions and ensures the person is more likely to get care that matches what they want. It can also reduce stress and disagreements among family members.
The EARLI program provides an opportunity for people to start conversations about their wishes at an earlier stage, through the sharing of their life story. Community aged care clients can also think about what matters most, and how their values guide their future choices. EARLI looks at whether sharing life stories can help people to feel better about their future health care. People taking part receive 4-5 home visits to guide them through creating a life story document and receive education on advance care planning, including Guardianship and Advance Health Directives. At the final visit, people can invite their aged care provider and their General Practitioner to attend to hear them express their wishes.
