At Home Aged Care Services Are Becoming The Support At Home Program in 2025

The Support at Home program is a new aged care initiative launching in 2025, set to replace existing Home Care Packages from November 1, offering a simpler and more flexible way to get the care you need.

At Brightwater, we offer personalised home care services in Perth designed to help you or your loved one remain independent at home for longer, with the right support in place.

You can pick and choose the services that work best for you, depending on the level of support you’ve been approved for. Below are some of the services your funds can cover:

Your own coordinator

At Brightwater, we’re here to make managing your support at home easier. From day one, you’ll have a dedicated coordinator who’ll get to know you, understand your goals and help you make the most of your funding.

Whether you’re focused on your health, happiness, or making your home more comfortable, our services cover all the essentials — plus a few extras you might not have thought of!

Your coordinator will:

  • Design a support plan with you that fits your lifestyle

  • Work with our health professionals to meet your needs

  • Keep your plan updated if your needs or goals change

  • Be your main point of contact for any questions or concerns

How much does Support at Home cost?

With Support at Home, you only pay for the services you use (plus the 10% care management fee). The cost of your care will depend on the services you choose and your personal financial situation.

Your budget

After your assessment, you’ll receive a quarterly budget from the government based on your care needs. This budget helps fund the support and services that matter most to you.

There’s also additional funding for longer-term supports like assistive technology or home modifications, so these won’t reduce your regular care budget.

If you don’t use the full amount, you can carry over leftover funds each quarter – up to $1,000 or 10% of your budget, whichever is more.

Care management

This is the work we do behind-the-scenes to keep your care running smoothly, including:

  • Create and update your care plan
  • Check in on your health and needs
  • organise and adjust your services when needed
  • Help you access extra support if needed

This way, your care stays flexible, personal and perfectly fits your lifestyle.

Costs and contributions

Based on your income and assets, the government may ask you to contribute to the cost of your independence and everyday living services.

Contribution rates are set as a percentage of the price of each service. The percentage contribution to the cost of services is:

How to get started with us

Once you've received your Support at Home letter of approval, just give us a call on 1300 223 968. We'll talk you through your funding level and explain the range of support services available.

From there, we’ll meet with you to go over the details, complete any paperwork, and start shaping a care plan that suits your life. Together, we'll work out what kind of help you need, how often you need it and the times that suit you best.

With everything in place, we’ll get your support underway so you can start enjoying the help you need at home.
