Advance Care Planning Support Service

Would you like some practical help with your Advance Care Planning?

We are pleased to offer a joint initiative between Palliative Care WA and Brightwater to help community members take the next steps in completing their Advance Care Planning.

The Advanced Care Planning Support Service can help you to:

  • identify what you want for your future planning
  • complete your advance care planning documentation
  • work out how to talk to your loved ones about your future care wishes.

One-to-one Advance Care Planning Support Service (ACPSS) sessions will be held on Fridays at Brightwater Inglewood, 2A Walter Road West, Inglewood.

Allocated booking times are shown below. Please note, if booking as a couple, or you require support with more than two documents, a double booking is required.

It is preferred that participants have attended an Advance Care Planning education workshop before using this service. If you haven’t yet attended an education workshop, please see Palliative Care WA's events section to find one suitable for you.

Please note: The ACPSS does not provide any legal, financial or health advice and is not able to arrange the witnessing of documentation. Individuals retain responsibility for anything written or decisions expressed within their documents.

The ACPSS is funded by the WA Department of Health as a pilot project in 2025.
