  • Explore activities, services, and social connections that are important to you
  • Connect you with both informal and formal supports
  • Coordinate your support and follow up when your needs change
  • Guide you in selecting and implementing your services on your own

If Support Coordination is listed in your Plan, it means you can choose a Support Coordinator to work alongside you as you get started and continue using your NDIS Plan.

    If you are not eligible for NDIS funding, you may be able to access funding from other sources or self-fund your services.
